U.S. Preventive Services Task Force Recommends Regular Exercise as the simplest strategy to Reduce Fall Risk

In the June 2024 edition of Scientific American, the article How Older Adults Can Exercise to Reduce Their Risk of Dangerous Falls recommends low-impact workouts that engage the muscles used in daily living. Does this sound familiar? We hope so! Every strength exercise in Chapter 7 of your EF Instructor Manual starts by stating the purpose and what activities of daily living the movement supports. America’s Health Ranking’s 2024 Senior Report shows older adults have made progress in measures of social connectedness and access to clinical care but struggle with socioeconomic barriers. The healthiest state for older adults was Utah. This was followed by Colorado, Vermont, New Hampshire and Minnesota. How does your state rank?

Evidence-based programs are improving the health of older adults in King County

Seventy percent of older adults in King County, WA have one or more chronic conditions such as diabetes, arthritis, or hypertension. Significant disparities exist in the prevalence of these conditions based on race, ethnicity, and geography.

The Veterans, Seniors, and Human Services Levy (VSHSL), from 2020-2023, funded 14 community partners serving diverse and multi-ethnic populations to offer culturally specific health and wellness programs that are backed by research and community-developed to address identified needs. Health interventions helped seniors, especially those from BIPOC (Black, Indigenous and People of Color) communities, to improve their physical and mental health.

Programs included chronic disease health management strategies, fall prevention and exercise classes, health education, and mental health counseling, and were provided in over 19 languages at senior centers, low-income housing sites, assisted living facilities, and home visits.

The health promotion programs reached more than 4,600 older adults, with 84% of surveyed participants stating they experienced improvements in health and wellness.

Health promotion programs backed by research

VSHSL partners implemented evidence-based and evidence-informed programs. Evidence-based programs are federally designated programs that have been rigorously tested in controlled settings, proven effective, and translated into practical models. Evidence-informed programs don’t have a federal designation, but they incorporate practices that have been proven effective through both traditional research methods and community expertise, and they are designed to align with specific community contexts and needs.

As part of the Senior Health Promotion investment, the VSHSL funded Sound Generations Project Enhance Team and the University of Washington’s (UW) Health Promotion Research Center (HPRC) to support funded providers in implementing evidence-based or -informed programs in their communities, as well as help make connections between providers, and to infrastructure and resources. Sound Generations and UW HPRC have partnered for three decades in developing, implementing, and scaling multiple interventions proven to improve older adult health and well-being.

Sound Generations and UW’s HPRC created the VSHSL Senior Health Promotion Investment Technical Support Team Final Report, to share the impact of and learnings from the 14 Senior Health Promotion programs that served King County’s seniors from 2020-2023.

Celebrating Older Americans Month


Reduction in Sitting Time and Blood Pressure in Older Adults

You may have seen a recent randomized trial to reduce sitting time and lower blood pressure in older adults in the news. Reducing cardio metabolic risk in older adults is very important, and time spent sitting is an opportunity for behavior change because we’re a sedentary society. According to Medscape, “…among older adults, on average, more than 70% of time is spent sitting during our waking hours. Observational studies suggest that longer sitting time is associated with increased risk for cardiovascular disease, type 2 diabetes, and all-cause mortality.”

Recently we were able to discuss the study with Principal Investigator Dori Rosenberg, PhD, MPH, Senior Scientific Investigator at Kaiser Permanente Health Research Institute of Washington. Dr. Rosenberg shared, “Our findings are really promising because sitting less is a change that may be easier for people than increasing physical activity, especially for older adults who are more likely to be living with restrictions like chronic pain or reduced physical ability.”

And she asked us to pose two questions to our Project Enhance Community: How could the sitting less approach help you and your community? What kind of program would you want to help you sit less?

The U.S. Office of Management and Budget published an update of the Statistical Policy Directive No. 15 on the collection and reporting of Federal race and ethnicity data to better serve a diverse America. Keep an eye out for updated EF and EW Participant Information Forms (PIFs) in the future.

Celebrating Older Americans Month

Every May, the Administration for Community Living leads the nation’s observance of Older Americans Month (OAM). The 2024 theme is Powered by Connection, which recognizes the profound impact that meaningful relationships and social connections have on our health and well-being.  

Sound Generations is celebrating Older Americans Month all month long by focusing on the power of connection in three of the many programs we offer: Caregiver Support, Community Dining and EnhanceFitness. Stories highlighting implementations at IDIC Filipino Senior & Family Services in Seattle, WA, and Sno-Valley Senior Center in Carnation, WA, will be shared throughout May. Follow along!

National Public Health Week


Implementing EnhanceFitness in Churches

In Hawaiʻi, kūpuna (older adults) are among the most valued members of society. Even the word kūpuna, by definition, denotes someone who is wise and highly respected, a keeper of knowledge and a living treasure of our community.

With support from the Atherton Family Foundation, Pearl City Community Church’s (PCCC) Kūpuna Empowerment Working Committee, YMCA of Honolulu and University of Hawaiʻi have published a handbook focused on planning, launching, delivering and sustaining EF in faith institutions to lessen falls risk among older adults.

EF4All Pilot and Next Steps

This modified EnhanceFitness pilot class was offered for 8 weeks to people with memory loss and their care partners at Northshore Senior Center in Bothell, WA, between December 2023 and February 2024. Participants completed pre- and post-program fitness assessments as well as self-report of satisfaction, and validated measures of depression, memory and cognition. A total of 16 participants enrolled.

More information on the participants’ likes and recommendations can be found here.

Next steps: Conduct a study with a larger, diverse group of participants to understand EF’s impact on health outcomes of this population.

Click here: Survey Opportunity – Preparing for the National Fall Prevention Plan Update

The National Council on Aging is planning a National Falls Prevention Summit, bringing together stakeholders to update the 2015 National Falls Prevention Action Plan. The event (by invitation only) is scheduled for September 2024 in Arlington, VA, and is funded by the U.S. Administration for Community Living. NCOA is inviting key stakeholders like all of you to complete a survey aimed at capturing progress, gaps, and opportunities regarding older adult falls risk, falls, and fall-related injuries. The results of this survey will be used to inform the National Falls Prevention Summit agenda and build a foundation for updating the National Falls Prevention Action Plan.

The deadline for completing the survey is Wednesday, March 27, 2024.

The survey should take no more than 20 minutes to complete. The anonymized, aggregate results will be shared with survey respondents who choose to share their contact information in the survey. Questions? Contact Emily Nabors at NCOA.

EnhanceFitness Strengthens Instructors Too!

From Mary Holland Branning, EF Instructor, Maryland:

“Testimonial to the importance of our EnhanceFitness Classes. ME! I had a total left hip replacement yesterday. I have been an EF instructor in two local senior centers since 2018, teaching back to back classes each 3x per week. I’m 65.

THIS is why we teach it, and why our participants need it so badly. I am using EVERY upper body muscle we work on to push out of the chair to use the walker. I am engaging my core, proper posture before I attempt to stand to take added pressure off of the legs to totally support me. Then before I sit down as well. The leg work has kept me strong and the doctor is very pleased at the strength in my legs previous to the surgery and says it will in no small way enhance my recovery.

I’m doing everything I ‘harp on’ in class over and over again, regarding the importance of staying strong overall. It has definitely reinforced this for me in these last 24 hours.

I can’t wait to get back to my classes to share this personal journey. I have wonderful participants including my 86 year old mom!

Increasing Physical Activity Access

Special Olympics, Inc. is part of the movement to improve the physical and social-emotional well-being of people with intellectual and developmental disabilities (IDD) by increasing their inclusion in healthcare, health promotion and disease prevention systems. Systems Change in Inclusive Health Subgrants help support organizations that are identifying solutions to systemic problems impacting the health and well-being of people with IDD.

The American Council on Exercise will develop curriculum to train physical activity leaders, both with and without IDD, to provide fitness access and education in under-served communities across the country.

Sound Generations will revise and adapt EnhanceFitness to improve accessibility to people with IDD, expanding national access to inclusive fitness programming.

When we talk about increasing access, it doesn’t only mean locations. It means access to knowledgeable exercise leaders that understand how to include people with Intellectual Disability (ID) in fitness opportunities. Check out a FREE Continuing Education class from our partners at American Council on Exercise to support growing your skills: Special Olympics Inclusive Fitness Online Training. In this interactive course, you’ll explore the unmet health needs, health disparities, and access barriers people with ID experience. Then learn principles and strategies for effectively communicating, instructing, and motivating individuals with ID. You’ll also review methods for inclusion, activity modification, program design, and fitness assessment. Additional Special Olympics resources are also be provided. Share your experience with other instructors in the private EF Instructor Facebook Group!

Requesting Public Comments for Falls Prevention Data Collection Tools

Some Enhance Fitness affiliates have participated in data collection for Administration for Community Living (ACL) Falls Prevention Grants in their state. If your affiliate is one of these, you may be interested in providing feedback about revisions to the data collection forms for future grantees. ACL has announced an opportunity for the public to comment on the data collection requirements for the ACL Evidence-Based Falls Prevention Program Information Collection. This notice solicits comments on the proposed extension of this data collection. Grantee agencies through this program represent a variety of organization types, including state agencies, area agencies on aging, universities, nonprofit organizations, and tribes. The respondents for this data collection are grantee staff, local staff, and program participants.

Comments on the collection of information must be submitted by Monday, February 12 at 11:59 p.m. ET.

January is National Glaucoma Awareness Month

Glaucoma is a leading cause of vision loss and blindness in the United States. But half of people with glaucoma don’t know they have it.

Glaucoma has no early symptoms. The only way to check for it is to get a comprehensive dilated eye exam. There’s no cure for glaucoma, but starting treatment as early as possible can help stop vision loss.

Anyone can get glaucoma, but some people are at higher risk, including people who:

  • Are over age 60
  • Are Black/African American and over age 40
  • Are Asian American
  • Are Hispanic/Latino
  • Have a family history of glaucoma

This Glaucoma Awareness Month, join the National Eye Health Education Program (NEHEP) in encouraging everyone at higher risk for glaucoma to get a dilated eye exam. It’s the best way to protect their vision!

Being HIV-positive is no different from being HIV-negative when it comes to exercise. Regular physical activity and exercise are part of a healthy lifestyle for everyone, including people with HIV.

World AIDS Day
Every year on December 1, we observe World AIDS Day to show support for people living with HIV/AIDS and work toward our goal of ending stigma and increasing HIV testing. This year’s theme is “World AIDS Day 35: Remember and Commit” — check out HIV.gov for resources to raise awareness in your community. You can share MyHealthFinder materials to encourage people to get tested for HIV and start a conversation with their doctor about testing. Finally, Healthy People 2030 offers resources related to preventing sexually transmitted infections.

According to the American Heart Association, “Clinical trials indicate that the practice of gratitude can have dramatic and lasting effects in a person’s life. It can lower blood pressure and improve immune function. . . . Grateful people engage in more exercise, have better dietary behaviors, are less likely to smoke and abuse alcohol and have higher rates of medication adherence.” Check out their how-to infographic and share with clients and coworkers!

Reflections on ASA RISE

On December 5th, the American Society on Aging (ASA) will announce the third cohort of fellows accepted into ASA RISE, a 20-week fellowship program for rising leaders of color in aging, already 45 alumni strong. Just two years ago, ASA RISE was still a concept and Sound Generations’ Deidre Daymon was selected as a fellow of the inaugural cohort:

“Being a part of the ASA RISE program has been a wonderful experience for me. This program recognizes the importance of addressing social injustice and has given me the courage and opportunity to not only share my own story but also to amplify the voices of older adults I have the privilege of working with. Health coaching in primary care has been shown to improve physical activity and dietary habits according to data presented at the Lifestyle Medicine Conference in Denver, CO earlier this month.

The latest health coaching CPT® medical billing codes have been updated in the Project Enhance Affiliate Resource Portal this month. Our understanding is that codes should always be paired with a diagnosis to be utilized by qualified practitioners.

More than 1 in 3 adults in the United States have prediabetes—and many of them don’t know it. National Diabetes Month is a time to raise awareness about diabetes as an important public health issue and encourage people to take charge of their health. This year, the National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) is focused on taking action to prevent diabetes-related health problems. Explore NIDDK’s tips to manage diabetes and its National Diabetes Month toolkit. You can also take a look at our MyHealthfinder diabetes page, which has resources you can share to learn more about preventing diabetes. Be sure to check out Healthy People 2030’s list of evidence-based resources related to diabetes that you can use in your work.

Special Olympics funds Inclusive Health Work

Special Olympics, Inc. is part of the movement to improve the physical and social-emotional well-being of people with intellectual disabilities (ID) by increasing their inclusion in healthcare, health promotion, and disease prevention systems. Sound Generation’s EnhanceFitness (EF) program is honored to be a 2023 grant recipient. Our adaptation project will support EF delivery organizations in including adults with intellectual disability and their supporters in EF programming with intention. An inclusive learning environment and safe, modified exercise moves will be provided as needed to include all abilities. Instructors will learn general teaching strategies to accommodate individuals as well as learning about inclusive practices generalizable throughout all health promotion and fitness programs. New CDC data shows the prevalence of obesity shot up across all states in 2022. With excess weight raising mortality risk, and obesity-related cardiac events tripling over two decades, that’s a heavy burden. Dr. Karen Hacker, director of the National Center for Chronic Disease Prevention and Health Promotion, says the solution is no secret: 

“We know the key strategies that work include addressing the underlying social determinants of health such as access to healthcare, healthy and affordable food, and safe places for physical activity.”

Deadline October 23rd: National Parks and Recreation Funding Opportunity

NRPA is pleased to announce a new grant opportunity, the Supporting Healthy Aging Through Parks and Recreation (SHAPR 3.0) grant — aka instructor training grants.

What are instructor training grants? These are grants that allow you or someone at your agency to become trained and certified to teach one of CDC approved, evidence-based physical activity programs for older adults in your community. Learn more and read in NRPA’s OpenSpace Blog what local P&R professionals share about their instructor training grants and how it impacted their community. 

The deadline to apply is October 23, 2023.

October is a GREAT Time to review your PT-REFER Partnership TOOLKIT!

The University of Washington Health Promotion Research Center and YMCA of the USA, with support from Sound Generations, developed this Toolkit as part of a research study (Physical Therapists—Recommending Enhance®Fitness (EF) to Expand Reach, or PT-REFER).
The Toolkit was designed for EF organizations interested in building partnerships with physical therapy clinics. It provides step-by-step guidance, in the form of specific tasks and activities, for engaging physical therapy clinics in
EF referral relationships. The Toolkit is available for download in your free EF Affiliate portal.
