EW Counselor Manual Supplement: ‘Providing EnhanceWellness to Adults with Long-Term Disability’ is now available!

The long-awaited EW Counselor Manual Supplement on Providing EnhanceWellness to Adults with Long-Term Disability is available for download on WellWare. EnhanceWellness (EW) is a program originally designed to serve older adults with chronic medical conditions. Our EW clients are older adults who want to make positive changes to improve their health and participation in all life has to offer. This supplement will provide general information about individuals with long-term disability and includes definitions of disability, medical conditions often associated with disability, disability discrimination and rights, and recommended adaptations to the EW process and program tools to better serve these clients. The focus of this supplement is individuals with long-term physical disabilities. However, some of this information may also be applicable to individuals with long-term mental, intellectual/developmental, or sensory disabilities.

The contents of this supplement were developed under a grant from the National Institute on Disability, Independent Living, and Rehabilitation Research (grant nos. HB133B080024 and 90RT5023-01-00). NIDILRR is a Center within the Administration for Community Living (ACL), Department of Health and Human Services (HHS). The contents of this supplement do not necessarily represent the policy of NIDILRR, ACL, or HHS, and you should not assume endorsement by the Federal Government.

The Age-Friendly Health Systems initiative is funded by The John A. Hartford Foundation and led by the Institute for Healthcare Improvement, in partnership with the American Hospital Association and the Catholic Health Association of the United States. An age-friendly approach will measurably improve the quality of care for older adults and optimize value for health systems. An Age-Friendly Health System is a health care system in which:

  • Older adults get the best care possible
  • Health care–related harms to older adults are dramatically reduced and approaching zero
  • Older adults are satisfied with their care Value is optimized for all—patients, families, caregivers, health care providers and health systems

See how EnhanceFitness, EnhanceWellness and other evidence-based programs are part of the solution:
