About evidence-based health programs for
older adults
If you would like further information about evidence-based health
programs for older adults, the following websites may be useful:
American Society on Aging
is the largest organization of professionals in the field of aging
and is geared toward enhancing the knowledge and skills of people
working with older adults and their families.
Center for Healthy
Aging (CHA) was founded in 1976 in response to unmet needs
for health services for older adults in the community. Since then,
CHA has become a nationally recognized agency for developing effective
models of care for seniors and their caregivers.
Healthy Aging Research
Network (HAN) is funded by the CDC and envisioned to assist
with the development of a research and dissemination agenda related
to the public health aspects of healthy aging. The nine universities
participating in the HAN Texas
A & M University; University
of California, Berkeley; University
of Colorado; University
of Illinois, Chicago; University
of North Carolina at Chapel Hill; University
of Pittsburgh; University
of South Carolina; University
of Washington; and West
Virginia University are a subset of the 33 Prevention
Research Centers located throughout the United States and provide
HAN with the area expertise necessary for its success. As with all
PRC research programs, there is a strong focus on partnering with
community- based groups to develop programs that improve health, with a
special emphasis on those communities and populations that bear a
disproportionate burden of illness and disease.
Living
a Healthy Life is a workshop for older adults with chronic
health problems. In the past 20 years or so, the Stanford Patient
Education Research Center (formerly the Stanford Arthritis Center
Education Office) has developed, tested, and evaluated self-management
programs for people with chronic health problems. Their first program
was the Arthritis Self-Management Course (also known as the Arthritis
Self-Help Workshop), which became the prototype for all their
self-management programs.
National Council on the Aging
(NCOA) founded in 1950, is a national network of organizations
and individuals dedicated to improving the health and independence of
older persons and increasing their continuing contributions to
communities, society, and future generations.
NCOA Center for Healthy
Aging is a national resource center for aging service providers,
Administration on Aging Evidence-Based Disease Prevention Grants
Program grantees, and those interested in healthy aging programs.
RE-AIM
provides resources and tools, including calculators for assessing reach
and adoption, for community leaders and researchers interested in using
the RE-AIM framework. Other tools include links, figures, tables,
presentations, measures, and publications.
U.S.
Administration on Aging (AoA) has launched a new grants
program and public/private partnership in support of community-level,
evidence-based prevention programs that have proven effective in
reducing the risk of disease, disability, and injury in older adults.
